New Client Request

If you need a copy of your clinical records or would like records sent to another provider, we're happy to help.

Current and Former Clients
Please contact our office by phone, text (573-239-9915), email (office@comoclinic.com), or your client portal to request records. We will send a Release of Information form to your secure client portal for completion. Once we receive the completed form, we will process your request and provide the records as authorized.

Healthcare Providers and Other Offices
If you are requesting records for a mutual client, you may fax your signed Release of Information form or records request directly to our secure fax at 1-573-340-1028. Please ensure all required information and authorizations are included to avoid delays in processing.
